We don't know yet if the cancer track will happen or not. If it is happening, it can be a rehash of Raman's sacrificing/airport track. Raman at the end won't let Ishita go away from his life. And they might show the cancer thing was a made up by Shagun, Subbu/some other villains or just an error in the medical report.
And, it might sound ludicrous but I won't discard the possibility of Shagun getting the cancer disease (maybe faking). In that way, it will be easier for her to get sympathy form IshRa and their kids.
