What the extra silver linings that it is allowed?
Scientifically, no biggies problem in marrying them. Genetics and health-risks thing occurred mostly if you married to your own sis/ bro by same paternal due to.
The risk of giving birth to babies with genetic defects as a result of marriages between first cousins is no greater than that run by women over 40 who become pregnant . Full article from The Telegraph based on research for that taboo.

It is not compulsory, but it is an option.
First cousin marriage is not something exclusive to Muslims. Many South Indian communities also have a tradition of first cousin marriage. I have a few relatives myself who married first cousins. The practice was common in many Christian communities as well. That is why you get jokes about rednecks and southerners marrying their cousin.
Back in the day people were unaware of the biological effects of in-breeding. A first cousin was seen as someone who is not a blood relative. The marriage was seen as a way to keep the family close knit and the wealth and property within the family circle. Also way back in the day life was confined to small villages. So clans and communities married within themselves.
As we learned more about negative effects of in-breeding and became a wider more open global community, first cousin marriage ceased to exist becoming even taboo or illegal in many parts.
