My guess is that the two changed their names when they introduced themselves to each other just for fun not knowing the other person had changed his/her name too.
When they finally decided to confess to each other and express their love, the accident must have happened.
Naren probably thought Pari died in the accident and went crazy in grief. Pari on the other hand probably woke up in the hospital and couldn't find Naren. She probably thought he left her without even saying anything.
Since they didn't use real names, Pari probably couldn't find Naren and Rushali couldn't track down "Pari" since she was looking for "Ahana".
What will probably happen is that Naren will see the real Pari and get cured. Ankita will probably decide to sacrifice her love for the sake of Naren's happiness. Ultimately, I think Naren will understand how much Ankita has done for him and will stay married to her.
Pari on the other hand will realize that Naren didn't betray her and that love isn't as bad as she thinks. So she'll be able to let go of the past and be open to loving another guy.
