Why are the D's taking advice from a medical student who calls herself "Dr"? It seems like she still is studying which is why she has this job to pay for her education.

Don't analys this serial in too much detail. When watching serials and movies, we need to put our brain aside. We watch it for entertaining and to stay away from our day to day crap. We don't want to see reality on TV or Cinema.
