I think in Murree, Mehru will fall ill and be hospitalised. Whereas Amal will attempt suicide or fall ill and be hospitalised in whichever city she is in. They are obviously shooting for both hospital scenes together here. They wouldn't have shot hospital scenes in Murree because the logistics would be difficult.

We can rule out Shanno not being in love with Mehru with the hospital scene. His worry is palpable and I think that scene is brought in exactly for this reason, to show Shanno is not faking his love. We are going to see how worried he is going to be about Mehru. Also MEhru ain't gonna die, time frame doesn't match, there are still more many more episodes to go.
