1) He cannot confront Dhruv about the questions because he has always placed Dhruv on a pedestral and he cannot imagine that his protection has suffocated the pfirend who he promised to never fail for. It's his own guilt and his love for Dhruv that is preventing him from asking these questions.
2) We know Manik is enabling Dhruv but he doesn't understand that. It's his defense mechanism and one that the whole fab5 used all this while. hence it will take a while and something major for Manik to stop this behaviour. He can never go against Dhruv in his head because he owes his life to him. Manik is nothing if not for Dhruv , he feels this debt and is humbled by it .Hence he has always tried giving up or making it to Dhruv in some way or the other. Also he might have felt responsible for Dhruv's attacks because that night Dhruv saved him and probably got this disease. Guilt.
3) Manik is shocked and numbed that all this while his friend rather brother was carrying this resentment. It's not about truth or lie, it's about what Dhruv is feeling or felt all this while and Manik thinks sad and is shattered that he was the cause of his friend's distress.
4) Interesting question.
First Nandini is the only one that he was real to him without any agenda or any sort of give and take. With fab5 while there is love but somehow Manik ended up giving himself to show his gratitude to their presence in his life. But with Nandini it doesn't work like that. It was love, the one he defined as based on absolute trust . He is vulnerable and stripped of any mask infront of her. Actually it's not that he is upset with Nandini but he is afraid of what Nandini actually thinks about him. He takes Nandini's words as God's truth , so when she said that maybe , it pushed him into an identity crisis. He told Nandini that he cannot kill, he is not that monster and he is disappointed that even with him being all real didn't make her see him as someone incapable of that cruelty. It's human nature nobody thinks they are capable of kiling and here toh he didn't .
Now for difference- Nandini and his relationship is of equality. She is his mirror of sorts . So it;s all about healing himself and having her still love him gives him confidence that he is good and that he is someone she will love and trust.
With Dhruv , he doesn't feel his buddy has erred anywhere, infact he feels he has failed as a friend and because their relationship is based on a debt where DHruv saved his life , manik will forever be grateful and accept anything as long as he has Dhruv around him, happy and content.
I don't think he is in a mental frame to compere these two situations. With Nandini, he knows that her doubt can shatter him completely and he cannot survive looking at himself. Also it's love a concept he doesn't know willl sustain in his life . He feels love should be above doubts. He is idealist like that. It;s his growing up in that sense.
With Dhruv he is obliged by his duty as a friend to keep trying - this is also because Nandu gave him hope, hence now Manik is trying to fight for Dhruv too.Otherwise he resigned himself to the fact that he lost Dhruv. If not for Nandu's hope he won't be fighting for his buddy. He would have continued blaming himself and guilt tripping, silently agreeing to whatever Dhruv would say or want.
