The word dhyan from scriptures is translated into English as meditation. It is a very important stage in yoga. Often people think that yoga means physical exercise. But, in reality, yoga means "attaining God". The word yoga literally means union. Here it refers to the union between individual soul and super soul, which is the same as saying that an individual soul has attained super soul.
People often call yoga as exercise because yoga helps one's health. But, still, achieving good health is not the ultimate aim of yoga. Let me take one analogy. Suppose you want to join your school's football's team. You know that you need to be fit for playing football. Therefore, you work out daily, you take healthy diet etc. When you do such things, then your health will definitely improve. But improving health is not your ultimate aim. Your ultimate aim is joining your school's football team.

Likewise, when people do yoga, then they often get good health. But the ultimate result of yoga is attaining God i.e. moksha or liberation.

For doing meditation properly, you should have good health. Therefore, good diet and cleanliness is very important. If possible, fix a room for meditation. This is because meditation produces positive vibrations. These vibrations will exist in the room in which you do meditation and these vibrations help in further meditation. As you continue to do meditation, amount of positive vibrations will go on increasing and further meditation will become easier.
It is not always possible to have separate room for meditation. Therefore, it is not a must. But, if you can, then do it.
Keep the room very clean. Keep those objects in the room, which make you feel happy and give peace. There are many things, which make us happy, but not all give peace. As an example, if you watch some suspense movie, then you may become happy. You may enjoy that movie. But it will not put your mind at ease. Rather, your mind will become restless. You will think a lot over that movie even after the movie is over. Do not keep those things, which make your mind restless. Rather, keep the things, which help your mind feel relaxed. Put pictures of gods, put some paintings, which show natural beauty (i.e.pictures showing greenery, rising Sun, etc.)
You have written that you get various thoughts during meditation. In the initial stage of meditation, it is quite natural to get thoughts. These thoughts will reduce with time and will ultimately stop altogether. To help reduce the thoughts, think of some mantra. If you do not know any other mantra, then choose "Om" as mantra. If you wish you can choose some other mantra. For example, if there is some god or goddess whom you worship daily, then you can choose a mantra related to that god or goddess. But whatever mantra you choose, stick to it. When you do meditation, then sit in an erect posture. Close your eyes lightly. The word lightly here is important because some people close their eyes very tightly, which is bad. Breath in slowly and continuously and then breath out slowly and continuously. During both breath in and breath out, concentrate on your breath and keep repeating that mantra. In between if you get any other thoughts, then just ignore those thoughts and continue with your breathing and mantra.

Whatver I have written in my previous post should be followed in the initial stage of meditation. Once you become good in this stage, then some modification will be required in further stage.

Oh yes, one question was regarding concentration and meditation. Some people think both are same. Actually concentration is one stage before meditation. Concentration helps in meditation.

Apparently it is easier to meditate if one does not eat onion and garlic, non-veg, consume alcohol etc.

Yes, satvik food helps in meditation. As a thumb rule, you can say that satvik food means food which is veg, is fresh, and is very little spicy.
If food is veg but is stale, then it is not satvik.
Onion and garlic are rajsik.
Alcohol is tamasic, because it clouds one's senses.

Food cooked over three hours ago is seen as Rajsik too.

I thought Onion and Garlic is tamsic, as it evokes emotions of anger, lust, jealousy, greed etc. Your knowledge of the shastras is greater than mine though, so I shall accept it's not tamsic and is Rajsik.
