There has been a constant mention of the baby being a boy by BK. I feel it has some consequence. P might deliver a boy and Sai a girl. Giving P another edge. We know the makers love for P. They barely make her lose big.

PS - I had not really watched episodes for a long time regularly. Has V ever apologised to Sai for letting the procedure happen? Or has he always been defensive about his actions?

It is possible. One egg can split into two - it is known as monozygotic twins. So ya a possibility of identical twins being born.
