You need to have a device called the TRP metre attached to your TV.This metre keeps a record of the shows being watched at the original telecast time.Repeat telecasts are not taken into account.
Dunno about current situation but only 800 houses in some 10-12 big cities have this metre.
They use this number to make a projection of the viewing pattern across India.
Has nothing to do with having a set top box.
If you have the TRP metre attached to your TV,then watching a program contributes to TRP.Otherwise it does not matter.Not at all.
There is possibly no other way to know whether a program is being watched without that device.
