Nothing to do with fitness. After plastic surgery to maintain procedure is a headache. She hasn't used her normal fillers in the cheeks after the artificially tweaked cheekbones..and shaved jawline. That's why they are sunk in so deep.

Look at her cheeks in Bewafaa, Yuva, Talaash ...they are full even though her body weight is same. Her eyelids have been gouged inwards. Those are the most apparent from her earlier films if you see on utube. Pictures can be morphed, the movies are trickier to change.
