1) Well it was never shown who added the prefix but I assume it was her too.. They didn't show it probably because the main part is "Baahubali" {the one with strong arms} because of her first interaction with the little one and it will be how he will be known. Amarendra was only used by some and on occasion too.. I think it was again she who named him Mahendra Baahubali.. We don't know.. but possibly because she wanted Baahubali to rule Maahishmathi.. his name should live forever kind of thing..
2) Maybe because she tried and failed.. and she probably thought he was not a big threat. At the end of the day, it was her words that became the rule.. so she probably didn't find him as a threat..
3) We don't really know if she did or not.. it may happen in next part.. Or she may have felt it unnecessary at an occasion like that.. 😕
