She was born Shikhandini. Wanted to be a man n kill Bhishma. Even wore men's clothes n married a woman like a man. The wife was shocked when she realised truth.
Then had a swap with a Gandharva to become male.
This is all I know as I haven't searched in the actual text for Shikhandi bits. Pun intended.
Texts state that Shikhandi was born as a female who fled to the forest and attained manhood after a humiliating episode post the revelation of his sex.
Shikhandi was referred to as the prince of Panchala because Drupada had hidden his actual identity. It was only after the marriage with a princess that the secret got revealed and prompted Shikhandi to flee. The latter then exchanged his femininity with a Yaksha and attained manhood.

So, during the war he was a man right? He lived all his life as a man, so why do they depict him in women's clothes? He was by body and nature a man
The things mentioned in the texts have been mentioned by others in the thread
Practically what would have been the case according to me is that he might have been a effeminate (or maybe Transgender/Gay). It was revealed to his wife only after his marriage and his wife felt cheated
He definitely had the capability to reproduce. But probably he was considered a woman in man's body
