If they wanna kill Akshara too then the best method would be to die during childbirth! As she was shown to have a complicated pregnancy history.

If both the males are dying, they wouldn’t keep Akshara alive

Yes that's the possibility she there must be some situation where one of either mother or child can be safe in such case she choose child and give it's responsibility to arohi and may die
