It's not about how it happened but about what he said. She meant everything to him and just before the accident he said she doesn't even exists for him from now on. It's the words and actions that pushes one to be guilty. Top it all he is doing the surgery if she looses voice he will blame himself for it too.

I get it but she was not concerned of what abhi said at that time, she was concerned of Arohi not doing weong and thats why she ran without thinking ans accident happen
