Yes this is what i'm also thinking that he clearly knows she is his wife. One more thing it can't be a blunder by cvs again & again. When pihu tried to runaway from home & ishita came to leave her pihu clearly called her ishimaa not once many times in front of raman& at that time he didn't question why pihu called her ishimaa. But some days ago when pihu called her ishimaa he did asked simmi why she called her like that. And i think he know only the part that ishita is his wife(may be runaway wife that too with ashok...the story which simmi told 😕).

Actually after seeing today's episode Raman's expression, look towards ishu make me thinks like that..
