IMO, it looks like an emotional outburst from Raman. I mean something on the lines of an emotional breakdown from Raman w.r.t losing Ruhi to Nidhi , as he thinks they will not be successful in getting Ruhi back within the given time frame.

Ishita might probably reassure him that she will not let Nidhi take away Ruhi at any cost and would definitely unite her with Raman & family, even if it means risking her life.

IMO, even the other day at the Agarwal house, when Raman read the letter, Ishita was saying that Mr.Agarwal has to be appreciated , for the great love he has for his wife. Despite the bigger problem he is in , despite all his tensions, he went to this extent to keep the promise he gave his wife. The words bigger problem, promise to wife were playing in my mind and was thinking if this would have a hidden meaning when Ishita is in trouble.

I somehow have a feeling that the discussion at Agarwal house, the goon attack and now this breakdown. all these things have a role to play out in Raman's mind when Ishita is in ICU.😊.

Not sure if I made any sense. I am only guessing.

For Raman - it is better not to expect anything - be it fair emotions, just treatment, equal POV, usefulness in a track, etc and then we can be pleasantly surprised like Ruhi's birthday gift idea that he had 😃
