I think that she was already having the affair with Ashok but then got pregnant with Ruhi. I think this is the point that Ashok used against Raman to plant the seed of doubt earlier that Ruhi is his daughter, not Raman's. Raman thought that another child would save his marriage with Shagun (he was clearly in denial), but Shagun didn't want the child.
