Because. Ishita can legally adopt Ruhi or Raman can file a legal application for the legal adoption. After which Shagun loses all rights to Ruhi and Ishita is seen as her Biological mother or an equivalent to that. And Ishita's name on Ruhi's reissued birth certificate because of a) ^ and b) As far as I know you need one parent to be genetically related to the child in a birth certificate if not both in case of surrogates or so.
