Chapter 3: Clashes


Early morning on the surgical floor:

Sanyukta arrived for the surgical rounds promptly at 6 in the morning. She had hardly gotten any sleep, as she was unpacking till 3AM. She was pleased to see Kaustuki on the floor also. Randhir gave instructions to the 2 medical students posted under him, "Sanyukta and Kaustuki, I want you to check on these patients and report to me in 15 minutes, as Dr. Jain is on his way"




Sanyukta was handed a list of 10 post-operative patient but there was no way she could check them all out in a mere 15 minutes
. She liked to interact with her patients and that meant spending more time with them. Nevertheless, she would try her best.



"Aapka dard kaisa hai, amma?" she sat down beside a middle aged woman who had just undergone a gall bladder surgery 5 days ago. "Beti, bahut dard hai, dawai se bhi asar nahin ho raha
. Tum nayi doctor ho kya?"



"Haan, amma main abhi padhai karr rahi hoon." Sanyukta replied as she checked her medication chart, "Amma, aap ko dard ki dawa tho poori mil rahi hai aur aapke operation ko bhi ab 5 din ho chuke hain" She sat down and started talking to her to find out more details
, "there has to be another reason" Sanyukta thought.




"Sanyukta, you're still here?! Dr. Jain has started making his rounds!" Sanyukta's conversation with the lady was interrupted as she sensed urgency in Kaustuki's voice.



"Main baad mein aati hoon, amma" Sanyukta got up and joined the rest of the team. Besides Kaustuki, Randhir and Dr. Jain, there were surgical interns and residents at the rounds. Sanyukta felt a little embarrassed at joining them late and when she looked up at Randhir, she saw him giving her the dirtiest look!



Dr. Jain enquired about all the patients but when there was no information available on 9 of the patients, he got restless, "Randhir, I don't expect this from you
. You of all people should be able to supervise your medical students properly!"




He walked over to the patient Sanyukta had been talking to, checked her incision and said, "Amma, aapka operation bilkul theek ho gaya hai, ab tho dard kum ho jaana chahiye
! Let's go team." Dr. Jain was about to leave when Sanyukta interrupted, "Sir, actually, this lady is very depressed, none of her famiy members come to visit her, and she's absolutely all alone in the hospital. I think that is why her pain is not responding to medicines alone. May be we should contact her family and explain that she needs them more than any pain medicine"



"You have a good point, Ms. Aggarwal! Hum surgeons ko yeh samajhnaa chahiye ki har patient ki taqleef ke peeche koyi aur dard bhi ho saktha hai, which can change their perception to pain
. Contacting her family members seems like a good idea, why don't you contact them after our rounds? Good job!" Sanyukta smiled, as she was grateful that Dr. Jain appreciated her extra effort.



She looked up at Randhir and saw him clenching his jaws in anger, as he ordered, 'Nurse, bed # 208 needs a psychiatry consultation' Sanyukta thought that was an overkill, all the poor lady needed was someone to talk to and a phone call to her son and daughter-in-law- NOT A PSYCHIATRIST! She decided to stay quiet, as Randhir had already given her enough dirty looks that morning.




"We need to hurry up, or we won't be able to accompany Dr. Jain in the operating room." Randhir warned Sanyukta and Kaustuki.



After the rounds, Randhir came over to Sanyukta, "You won't be coming into the operating theatre (OT). You will stay on this floor and do all the scut work, got it!? Here is a list! When I am back, you shall report to me. Here's my pager number." Randhir turned around and followed Dr. Jain and the residents, "Kaustuki, you can come along."




Sanyukta knew why she was being punished. Not that she was dying to go to the OT - she preferred awake and interactive patients than an unconscious, cut up body on the operating table! "This is going to be the longest month in my life", mumbled Sanyukta as she shuddered at the thought of facing Randhir's temper every day.




Lunch in the cafeteria:



Sanyukta and Kanika had just settled to eat when Sanyukta got paged, "Oh no! I'm sorry Kanika! I need to go."



"No problems, but at least have something to eat before taking off?"




"No can do! Randhir is already irked!" Sanyukta apologised




"Oh, you're working with Randhir as your senior? He can be a slave driver! Good luck. And a word of advice? Just do what he says otherwise he will make life miserable for you!" Kanika waved her good bye as Sanyukta ran with her belongings. As Randhir was more interested in being in the OT, he usually had his juniors do the running around, gathering patient reports, checking their vital signs, giving orders to the nurses. Most junior students preferred to stay out of his way. Sanyukta, being new was assigned to work with him for a month as part of her surgical rotation.



Surgical floor at the nurse's station:



Sanyukta walked in to see Randhir talking to someone on the phone, "Yes Vidushi. We'll go over it in the evening! See you at the hostel." Sanyukta figured he must have been talking to his girlfriend. She presented all the reports and case summaries to him as she had been ordered.




"Achha, where is room 203's biopsy report?" he asked



"Oh, Mr. Verma? Here it is. It looks like he has 3rd stage cancer." Sanyukta always preferred to address her patients by their names rather than room numbers.




"Yeah. Did you tell him about it?" Randhir asked rudely



"Me? How could I? It's major news, and I'm merely a 3rd year medical student!"



"So? How does it matter? Dr. Jain and I have no time for this. It is better that you get used to giving bad news at an early stage. How else are you going to become a doctor?" he smirked




"But this is big news for the patient. Wouldn't you rather that, in the least, a resident conveys it?" she asked.



"They are even busier! Don't ask too many questions! This is an extremely busy surgical hospital! Tumhare Delhi jaisa nahin hai yahan, farzi! You need to be more efficient here! Our job in surgery is to cut and cure! We don't have time for chit-chat- GOT IT?"




Sanyukta opened her mouth to proest but rememberd what Kanika had said and remained silent. She simply nodded and looked down at her notes. She knew this was going to be a very painful month indeed! How she longed to go back to Delhi or work in medicine or pediatrics, where people were more "human."



"What about the blood tests on that kiddo in 210?" he asked, grabbing her attention once more.




"You mean, Rohit? Erm, the lab lost his blood test results" she answered.



"So send his blood once more for tests!!!' Randhir said angrily; he was beginning to lose his patience with this new student!



"Yesterday's reports were all fine, were they not? What is a repeat of the same test going to prove? Do you think it will change Dr Jain's decision to take him in for surgery? The poor kid is scared of needles" she hesitantly answered.
Randhir got little flustered, "Uh..uh..do whatever is asked of you, understand, you silly girl. You're merely a third year! Don't think for even a second that you know more than your seniors!"



After he said that, he thought about what Sanyukta had said. She was probably right, there was no new information they would get by drawing more tests on that poor boy.



"Umm, Sanyukta, on a second thought, leave the boy alone" he said as he walked off. Sanyukta smiled and tried to catch up with him; she knew he expected that she follow him around everywhere while he was in the ward.



"Okay, I'm going back to the OT. You and Kaustuki need to meet me at 5PM sharp for rounds, got it?!" he spoke as he took the elevator. Sanyukta finally sighed as she could get a little break from Mr. Rude and Arrogant!



That evening, during rounds:



They had a bigger group of residents and students with Dr. Jain during the evening. These rounds were generally held at a more relaxed pace with Dr. Jain and the senior residents finally taking time to teach their students and interns some pearls of wisdom.




Dr. Jain was fond of quizzing his students and taking them by surprise. Randhir was usually very confident and had an answer for every question asked. But today for some reason, Dr. Jain caught him unaware as he asked him a difficult question, more related to basic sciences rather than clinical medicine. "Randhir, I expect you to know this. After all you should be preparing for your finals now!"




Randhir was thoroughly embarrassed, since something like this had never happened before. He looked up and saw Sanyukta scribbling something on her notepad and holding it upright behind Dr Jain. Randhir quickly read off what she had written. "Good job, Randhir. I knew you would know the answer to something as basic as that!" Dr. Jain patted his back and moved on to the next patient. Randhir just looked away sheepishly as this was the first time a junior student, and that too a woman had come to his rescue.




After the rounds, as the students were wrapping up the last details on the patient charts, Randhir came upto Sanyukta, "By the way, there was no need to help me, I knew the answer!"



Sanyukta rolled her eyes and turned around, "Randhir, I can understand. You're in your 5th year and thus, basic sciences are sometimes forgotten when you're at that level. I remember because I'm merely a 3rd year!" she threw his words back at him. "By the way, you are welcome if that is your way of saying thank you!" she turned around without waiting for his response and resumed writing in the chart. She thought, "why can't he ever accept that he is not perfect?" Randhir was a little embarrassed, as he knew that Sanyukta was right but his ego could not admit to it. He turned around and walked away.
