Garima is not Khushi's nor Payal's real mother
So i'm guessing the past would be
Arnav's dad and Garima might be lovers but Arnav's dad was forced to marry his mom by da idiotic dadi...
He might have moved on in his life... however, one day met Garima again... thus all da love starts again...
he might have told his intensions to his wife and she suicides...
However... a few qualities about Arnav's dad as mentioned in the various episodes
- he is too fast in deciding things (nani told ASR aft da raftar part whn devayani's character was introduced)
- he has high temper (Dadi mentioned it to ASR when he said he is not like his dad)
or may be Arnav is Garima's real son because Arnav and Anjali has a huge gap among them... Arnav was 14 and Anjali was to get married đ remember his flashbacks...
Ohh lord.. this suspense is going to kill me...

Thus my take...
Khushi has gone to get the medicine... On her way back she would see Anjali trying to get up, walk or cry...she would get concerned and worried so will quickly go towards her.. she would see things around and ends up getting stuck at the wires... Since the wires are opened near the stairs... she will fall down from the stairs...Anjali will be too shocked to do anything and Shyam will run away so none of the other family members see him there... Arnav will be having a confrontation moment with Dadi and Nani when he would hear Khushi screaming...or a loud noise... or people screaming... KHUSHIJI!!!!!!
