because Anu had (accidently) had the transplant
Anu went to doctor for daily check up and the nurse got the files mixed up and took Anu as the women who's gona have the transplant
then Anu had it n she doesnt know about it nor does nikhil that Anu is preggy coz he hasnt met that lady who'll give birth to "His child"

Hello Dear,

Annu and Nikhill doesnt have anything to do with it.
An artifical insemination had been done on her by mistake by the doctors.
when the insemination is done she gets pregnant, actually the insemination was for another patient, but doctors had a confusion and annu was inseminated.
