I would thank Mahima for sending all of them out and ensuring that only Abhi is near her. They need to talk it out to sort it out. She heard him even when unconscious. Like you said, they need an intervention who helps them see things clearly but correctly and lead them in the right path. All through they have only lived for others and they need that guidance who will help them reach their destiny that is LOVE (united). Here the nurse did the job, she was on point and I am happy nobody interrupted that convo especially Arohi wasn’t around there.
I understand where Manjari is coming from now. Harsh has always kept her engaged in some task so that she does not use her mind and robotically follow his instructions. She is not in a position to catch the fact that her son is doing this as a compromise for her health and happiness. But it’s not her fault completely. She has never learnt to take her stand against Harsh and never made it clear. And that’s exactly the reason why Abhi is so vulnerable.

I'm glad we got today's episode. How Abhi got the clarity, that being rough and angry with Akshu is not gonna take him anywhere, Akshu realised today thay Abhi's love for her is immense. And she loves him too, and this whole separation doesn't make any sense. Nothing makes sense. None of this. The only benefit of Goenkas not being around was this,that Akshu got some clarity. I thank the nurse from the bottom of my heart, that she cleared all of Akshara's doubts...
I wrote somewhere that I couldn't imagine in what state of mind a mother can respond to her child's cry with 'kuldevi mandir'. But, when I read your post, I realised, manjiri is robotic when it comes to life. She's just following whatever Harsh tells her. She wants his approval, and what he says is correct. That's why, even though it's right in front of her, she's unable to understand the depth of her son's pain. The only solace is that atleast she sees he's unhappy. But, idk, if Harsh stays against Akshara, I really don't see Manjiri being strong and taking a step in their support. She needs to get out of the toxicity of Harsh first, only then can she think of being there for her son.
Here's where I like Mahima, even though she has her own selfish motives, still she's never not acknowledged Abhi's love for Akshu. I wish she was a bit nicer though, because all the tilak confusion could have been avoided had she spoken out.
I sincerely feel that all these toxic relationships will slowly take a turn towards positivity, only when our Abhi and Akshu join hands.
