After thinking a lot I got a weird idea. Please spare with my rant 😃.
It might be Mandar's ex whom he dumped to marry Pallavi, the girl his family chose for him. So she might hold a grudge against them and hate Pallavi for snatching away her love of life. That's why she is targeting D fam especially VD. And now after seeing Pallavi leading a life with successful business tycoon like RR, the fire of her revenge started to burn once again.
She is framing VD to increase the distance between him and Raghvi. After separating them from D fam she might try to create a rift between them. And also she might be friend of Mansi or Amruta, that's the reason she knew about the internal fight going on there.

Second, this person knows the exact dynamics between Vijay Deshmukh, Raghav and Pallavi and how hurting Vijay Deshmukh can break apart the sensitive relationship of Raghav and Pallavi. Let's dissect it further:
Someone in the D family and someone in the Rao family is a traitor. Inside information is getting leaked to this outsider/villain.
From the D family, mansi seems to be a high possibility and if it's not her, the second possibility may be a completely different person who is paying two traitors to destroy just not Raghav but also Pallavi i.e. destroy their relationship.
Which brings us to another scenario, an ex flame of Raghav because Pallu had none. From what was brought up by Pallavi today about Raghav's past, it seems like something is resurfacing, rather someone from his past.
I'd rule out Mandaar because he cannot hurt his father and he does not need to play cheap games. He can just show his face and Raghav and Pallavi's marriage can easily fall apart as it is.
