I haven't seen the episode but this is quite a possibility. Sakshi was killed in an accident when he was driving so he blames himself. If Purvi had fallen to her death in front of his eyes, he would again blame himself for not being able to save his kids' mother...he probably had a flashback to Sakshi's accident
