many ppl address each other as bhi or behen, in general, just for respect, and if they get into a relationship only then they call names or something else, so this might be the case for Sandy..

and about NK's heart, well he has a heart, just that his brain over powers it, he loves the ppl who thinks like him and supports him blindly and sandy is one of them, and so he actually respects and cares for Sandy. and Sandy is using this on her favor and manipulating NK..
