There was no defining moment shown when KGH staff came to know about AshNi in love. Dr. Rangnath had perceived it and had remarked about it at several occasions: One, when Nidhi was sacked from KGH and was completely shattered, he had told her "Dr. Nidhi, mujhe andaaza hai ki kya hua hai".
Next day when Ashutosh had blasted a janitor pointing to trash and when Mallika had tried to intervene, she too had been chided by Ashutosh, saying nobody was doing favors to him by working in hospital and if anyone felt so, he/she could leave at that very moment; then Rangnath had remarked 'dono taraf aag babar ki lagi hui hai"
I think, once he had called Anji too and talked about Nidhi feeling heartbroken.
