I don't think Salman was joking either. If you look at the scene where Gautam was telling Sushanth to keep quite - his expression was indeed very "psycho-like". Its not necessarily about questioning his mental ability - its just another one of Gautam's nautanki avataars.

Personally I think it depends on who is using the word for whom & in what context. It can range from something light eg. "You are psycho man!" = "You are crazy man" to the literal meaning (i.e. doubting the person's mental ability.
