I think that Aahil could be upset either because, as others earlier said, Psycho Billo told him that Sanam intentionally drugged him OR (and I hope that this is the accurate one) Aahil remembers that he told Sanam about his past and he's afraid of what she thinks about him and will leave him now that she knows the truth.
