OH dont kill me for this guys, BUT i got the most wildest prediction....
Maybe RR made a deal with him that He would have to return back to sanjana or marry to sanjana if he wants prem's family to be happy and if he wants Heer to be happy...So for their happiness he has to return to sanjana and marry her (or has he already married her??)

OMG I hope this never ever comes true...but thinking the BT style...this could be the worst case scenario

yea exactly...i dont want any illness crap....he just got saved from teh biggest incident....why go bak to death again....i just hope before RR blackmails him to marry sanjana,,PRem will figure out sometihng else and also hopefully by then Heer would know everything and He will let her help him and together they Conquer the world of evil and bring peace and happiness to their fam and to their fans!!LOL

well i too second da fact that prem is not sick! even if he is sick heer will come to know about it and make ure that prem will be better for if he dies second time over then BT better be prepared to shut the serial down for i dont think any1 wud want 2 watch it!
