Phineas Nigellus is Orion's Great Grandfather unless you're talking about his grand uncle Phineas (middle name unknown) Black who was disowned for supporting muggle rights so to be safe I chose Walburga who also happens to be Phineas Nigellus's great-granddaughter and Orions uncle Phineas happens to also be Walburga's grand uncle.

Sirius and Regulus's parents were second cousins.
