Who decides a person is insane? professional in the field or feedback from loved ones or peole interacting?

Will go with feedback from loved ones or people interacting.. Professional most of the time say we had following test/questions and out of 50 questions /40 ere right and they conclude person is sane.. helloo there are treands in behaviour that is not normal and are observed based on situations, which in opinion are insane...
