They can say that it’s gender change and now they consider themselves as boy / girl
P.S. Babies are born with the gender.. M or F. Their reproductive organs is made at birth.. how can Doctors change it.. and make new body to suddenly create sperms or eggs? 
Intersex and Transgender people exist. Don't you know about our Hijra community? Most of them are Intersex or Trans people. Some XX females are born without an ovary, vagina or uterus (Müllerian agenesis), and they are still women. Having what kind of primary reproductive organ or absence of it does not necessarily make the brain feel the socially expected gender of a person. Most of us never think about what our gender identity is because it matches our sex assigned at birth.
Gender-affirming care is not about reproduction or an altered body to create viable sperm or egg that comes under fertility treatments. Surgeries are not done on underage trans kids; it's for adults. But sex-assigning surgeries are done on Intersex kids by the parents to fix them even before they reach puberty. Often, Intersex people get the surgery they don't want, and trans people don't get the surgery they want.
Transgender (Gender incongruence) occurs when a person feels different from the sex to which they were assigned at birth; the current estimation is 0.1% to 0.6% of the population. One hypothesis is that prenatal (before birth) hormones are responsible for these changes in the brain. Genetic studies are still underway.
Intersex is an umbrella term for many natural variations in sex characteristics. These variations may be chromosomal, gonadal, anatomical or hormonal, with current estimation making up 1-2% of the population. There are XX males, XY females, XXY, mosaicism with both XX and XY. The variations go on. That's why it's a spectrum.
