The spit hand shake custom is to spit in their OWN hand then shake hands - not other people's hands. It's usually done as a promise and is a watered (hah) down version of a blood oath where people cut themselves and then share their blood.

Gross but also consensual which Aamir spitting on women is not - whether he thinks it's a joke or an honor.
