It's possible that it was just an impromptu joke because Anushka said ADHM is a 'suspense romance' in response to Kapil's question. There will be some twist in the second half (Alizeh's sickness or Saba's past/future) but I doubt it'll be anything supernatural. That's just not Karan's arena.
