It is a reference I used to characterise akshu. Akshu is a smart, intelligent and strong girl yet she never used them to stand up to herself in front of aarohi. In the name of love for her sister and family she let aarohi bully her all these years. She thought aarohi loved abhi that's why all these time she backed away from abhi. But recent events gave her clarity that aarohi never loved abhi. First she comes to know MD thing and then aarohi going to exam on the same day. When asked aarohi told two things. First She told she will manage abhi then she doesn't love him maybe will love him in future. To her she is best abhi is best so both should get married. But abhi had always wanted only true love. Now knowing that her sister doesn't love abhi had hit her. Then akshu heard her mother's voice and mimi talk about how her mother always stood for truth even if people didn't understand first later she won. That put her in a dilemma to tell truth. If she did she has to go against her sister.

We know aarohi and abhi are same in zidd. One has good intentions and the other bad. Aarohi in her zidd pushed akshu to an edge that if anyone knows any truth she will blame it on her along with her other threats. Then abhi will come tomorrow and push akshu to another end by giving her ultimatum. Now she has two paths to go and she needs to choose one. Choosing abhi and her love benifits both and get their love but will make aarohi broken. Choosing aarohi means choosing lie and making everyone life miserable.

So it's technically akshu's " Kudh se kudh ka rishta " begins here. It's not about her choosing abhi over aarohi. It's her choosing herself, standing for truth which is what she is. Abhi asked her when will you love me like you love your family and sister in the beginning. This is what it is.

This is really good analysis. I really hope that the writers were thinking along the same line.

She cares but is a very selfish person in my opinion.
