There are ample subtle hints about that. Recently in the society parking lot, Ishita told Raman no matter what Raman do, Shagun would always be special for him as she gave him the biggest happiness of his life, his children. She even mentioned she would never be able to take Shagun's place in his heart. Raman felt her insecurity and diverted it saying they would make new memories together. Before that, they had a conversation after Raman's friends anniversary party where ishita clearly mentioned she could never give him the happiness that Shagun had given to him. She accepted that no man would love him considering her condition. It's got nothing to do with Raman being a crass or a saint, it's all about Ishita's biggest insecurity. This the reason after all Raman had done, Ishita still thinks Raman still loves Shagun. Deep inside her insecurity makes her vulnerable to progress further in their relationship.

As far as confession and conceiving are concerned, you've got it ALL WRONG. The comparison was drawn to clarify if their biggest insecurities,which were the reasons of rejection in their lives once, are used to force them to confess what IMPACT it would lead on to them.
