that Ishita will have surgery to fix her hand issue...and then will have to go out of town for rehab...maybe I am overthinking.

There is no rehab concept in India. At the most Ishita will have to go for physical therapy. Anyways, the importance of that scene is probably something different. Now a days, Raman wants to be there for Ishita all the time. I guess Raman will not be able to stay still and will reach the place where Ishita will go and will see her in the condition that Ishita "does not want Raman to see." It will be romantic.. and Raman taking care of Ishita.
