I believe Mrs. Bhalla suffered from vasovagal syncope. The vagus nerve became activated due to her fear (stress or some other factor) which caused 2 things to happen.
1) Her heart rate (and the contractility) decreased which caused the cardiac output to decrease. Which means not enough blood for the brain, which caused loss of consciousness.
2) the vessels dilated due to vagus nerve activation which caused the BP to drop, which caused the fainting.
Both of these things can happen, or one of them can happen but the end result is loss of consciousness.

The fainting could also have been brought on by hyperventilation. Mrs. Bhalla seemed to be hyperventilating. When you hyperventilate, you reduce the level of oxygen in your bloodstream and increase the level of carbon dioxide which can also cause fainting.

They might have actually done their research this time around. Or just gotten bloody lucky.

As for how Ishita knew the BP was low. I don't know but its a good guess. If a person is unconscious then their BP is not going to be too high. You cannot truly know the BP without checking though.

A lot of things happen at the physiological level that we are absolutely unaware of.
