It doesn't have to be miscarriage, the baby may get kidnapped. The scene came out of nowhere, Chakor got affected but Suraj changed her mood by talking about baby names.

oh good yes Suraj never believes in superstition and is always positive even during the godh bharai scene where that old bolt never allowed sukor to bless that brutus.

Sumi, Brutus is telling kn all about ranjana... That means before this scene something ranjana related should happen... Brutus mentions about ranjana getting some paper from suraj's room.. God alone knows what

I think Imli is cooking up a story to hide her intentions from KN. KN looks shell shocked, so I think either Imli told him that Ranjana got Manohar killed or she's claiming that Ranjana wants to kill Suraj.
