After reading this spoiler, I think only after the birth of karan the sandhya's position in chauhan house become less so she used karan as a target to gain family's attention towards her and thinks to rule over them and keeps everyone under her control. I hope soon meghna and naina should make sandhya real face come in front of everyone.

Swabhimaan:

Meghna and Naina are happy as Karan is changing and bonding with family. Sandhya recalls Karan's Naamkaran function. She tells everyone that she has done a lot for this family, and today they are raising questions on her place in the family. She thinks to make everyone realize her value. She is hurt as everyone started taking her granted. She decides to rule the house by making everyone depend on her. Meghna gives Sharda the house papers to return her self-esteem. Sharda does not accept Meghna's help. Naina asks her mum in law to go office and encourages her, but her mum in law does not agree.

After reading the spoiler the flashback of karan naming ceremony by sandhya i have strong doubt that sandhya is giving expired medicines for karan from his childhood. I don't think he is having any skin problem - only due to the expired medicines he is getting affected on his skin. As I have already mentioned that naina shouldn't believe the karan old reports and know nairan became friends and also sure meghna will help naina by that somehow they can opt for new medical checkup for karan with the help of kunal and vishal that too with out the knowledge of any one in chauhan especially sandhya.
