The helplessness, sorrow, shock and trauma that Ahem showed when Gopi was injured cannot be just guilt. If it was then he would not have tears in his eyes. If this had continued without the accident in between then we would have seen Ahem do more things to Gopi slowly working on the attraction and care to fall in love but the accident just fastened this. On that day Ahem saw what he would lose if Gopi is not there.
For a person who considered that his life was ruined due to the marriage losing Gopi should not have caused so much reaction.

But due to the shock and the unexpected reaction, he has now slowed down. A proud man, Ahem still does not want to accept that he may have been wrong before. Hence the dialogues of duty and guilt. I am not saying these are not there. Ahem feels guilt for sending her to the goons and not helping her just like he feels guilty for letting her fall. But that guilt will not eat him up like it did if he did not love her.
He cannot see Gopi in tears now. He wants to help her and share things with her. If this was the just concern then he would get a nurse, a very competent one or would have been satisfied with his mother's words.
Take for example the hair wash, so what if Gopi is unable to wash the hair that day. It can be classified as duty if when he heard her problem and when she said she was ok, he tell his mom or Meethi to help her later. But our Ahem did it himself.
