Indeed a WELL DESERVED slap! And the reason sultan didn't shake her hand was because he could not accept her congratulations as it wasn't really his birthday. This man has integrity. He might not know how he feels about madhu.. and he certainly thinks that their relationship is one of friendship but he isn't just going to touch for coz he feels like it.
In fact if you recall...when madhu confessed to him and blurted that she was lying to everyone...a few episodes ago and she broke down and started crying and turned around, sultan reached out to place a hand of humdardi on her shoulder and it was NEEDED for Madhu but he held back and didn't coz he didn't want to put madhu in any further feelings of discomfort. This man knows his maryaada.
