No, as far as i know, it was real, and he LIED to Khushi that it was all natak as he didn't want her to get anymore WORRIED, watch the scene between Anjali and Arnav again, she talks about it, as she saw the whole thing (where Arnav tells Khushi he faked being unconscious) and just knew Arnav was lying from past experiences. He was doing it so that Khushi wouldn't worry so much, basically, through care.
