I google searched to find the answer to your question ... and this is what I found...
Rome came to power and conquered a quarter of the planet in 1000 years before internal strife led to their downfall. Alexander the Great did the same thing without any backstabbing and it took him only 13 years. His father Phillip taught him how to be strong and he took it to the next level. The Persians crumbled under his rule. Athens and Sparta united and Egypt gave up on attempting to be of impirial status. Alexander is found on reliefs and stellas throughout Egypt from Thebes to Memphis and aside from Cleopatra he was the only Mesopotamian ruler to truly rule all of his empire. Hell, even the all too famous Julius Caesar had his governors and provinces that did his dirty work for him. Truly when you put all the pieces together, Alexander the Great was the most influintial ruler of the ancient world.

Alexander the Great was called that b'cos he conquered several countries and destroyed the primary power of that time - the Persian empire. His terretory stretched from Greece to Egypt, and then he overran Persia - the first time it was conquered in history - and then went on to unsettle the Western parts of India. Had his troops not mutinied, he'd probably have conquered northern India as well, and even the wilies of Chanakya and Chandragupta wouldn't have worked against him.
The list of rulers who have 'the Great' appended to their title are the tsars of Russia Peter I and Katherine, and in other countries, Charlemagne (founder of the Holy Roman Empire). There have been other titles appended to other rulers, like William the Conqueror who established Norman rule in England, Richard the Lion Heart, Suleiman the Magnificient, and so on.
