What they do I think is repeat the dialouge in Hindi so its ok.

In the South, marriages between first cousins, even mamas and nieces are permitted.

It is not uncommon for older generation Malayali women, even those who are not married to their first cousin, to call their husbands "cheta" which this the same as big brother, or Anna.

Tamil women call their husbands "mama" or "athaan" (paternal aunt's son or cousin brother).

Women call their husbands "Appa" (father) in front of their children.

I did not see the part you are referring to, but I think Subbalakshmi is calling "Anna" as a Title by which he is popularly known, not in the sense of big brother. Just like Baabi calls Dutta "Bhau" or big brother.

Only Nakku does not call him Bhau :-) Because she takes the meaning of that word seriously :-) She has no sisterly feelings for him :-)
