Khamoshi (Old One). I think this one had Waheeda Rehmaan as a nurse who treats Rajesh Khanna and before him she also treats Firoz Khan from being a Paagal to a normal man. Then in the process she looses her mind. Very sad and well made movie.

the saddest movie i know that hasn't been mentioned is saajan ka ghar with juhi chawla and rishi kapoor. juhi lead a horrible life throughout the movie which even ended very tragically.
