My guess is she roofied him once she felt he would inherit everything, got him trapped with the guilt and he had to marry her. Probably the pregnancy was also a reason.

Harsh was not adopted. Kesar simply placed him in the bassinet where Tulsi’s son was supposed to be placed. Ansh was kidnapped by Raj, the man who raped Kesar and fathered Harsh. Kesar couldn’t stop him, Tulsi was still unconscious owing to her delivery and so never knew a child-swap had happened. Not until something leads to a doubt regarding Harsh’s paternity.

Kesar can’t keep mum when it seems Tulsi was being suspected and her character was unfairly tarnished. She spills the beans to ensure her sister is not unfairly maligned for life and that’s when everyone finds out the truth about Harsh and they lay the groundwork for the introduction of Ansh.

I think makers remembered seeing the posts that Karan had a son Parth who was never shown and they were like wow ek aur character mila, jaldi lao usko... No reason on why and how.
