I think what she meant was that Karan finally got to experience parental love via her for the first time in his life despite the fact that he is Mihir’s son with another woman. Remember, Mandira kept pretending to have lost her mental balance to ensure Mihir stayed with her and she played with Karan in the same manner to ensure her son would hate his dad enough to go after him and get him to marry her. So she never did give Karan the kind of love a mother would, Mihir never accepted him wholeheartedly even though he ensured he did his duty by Karan but that was done as a bare minimum - he paid the bills for his schooling, clothes etc. probably showed up if Karan was seriously ill or injured if Mandira called him but nothing else. He didn’t have a warm relationship with Karan until Karan came home in SN and that was owing to Tulsi. Mihir didn’t accept him and allow him to be a Virani until then, he was avoiding Karan as much as possible until then as he was ashamed of himself for having fathered him.
Tulsi gave Karan the opportunity to experience parental love for the first time in his life. He was allowed to be a bit childish after he got to SN as we see him with Nandini which didn’t really show up before. We see once he starts living in SN there are a couple of scenes between him and Tulsi about his life in Australia, one of them is about Tulsi asking him about his food preferences, what he likes and dislikes. He tells her he finds the food a bit spicy; and this is Gujarati cuisine, spicy is not really a thing in our food but he’s lived in Australia and mainly survived his life on bread butter, pizzas and the sort. Mandira didn’t cook. They build up that relationship slowly and awkwardly. And it is because of this that Mihir allows himself to love his son.
They even included the flashback of the scene where Karan is shown as an adult in front of the mandir of SN where he revealed his true identity in front of her. So the idea is not to indicate raising him from childhood but rather to show it was once she accepted him that his son was allowed to know what the love of a mother and father is like beyond that of a family’s as well.

Are they changing the narrative of Mihir-Mandira affair? Earlier Mihir said that he was unconscious when Mandira's consummation happened. But in today's monologue, it looks like they were having full-fledged affair?

It seems they are or trying to do so, because earlier they implied that Mihir fathered Karan while he had lost his memory. Yesterday they had a flashback scene play out from the time Mandira was faking her pregnancy and pretended to faint at some event right in front of Mihir and Tulsi; Mihir goes rushing to her before she can fall down. But we know she wasn’t pregnant really at that time. They even play the scene where Karan reveals who he really is to Tulsi as well.
Tulsi had no idea Mandira was alive for nearly two decades let alone about Karan’s very existence until he showed up. The dialogue writers and the script writers need to sit down with the director and clear this out. Because canon clearly won’t change regardless of what they want now.
