for her, her nephew is also like her child. she has genuinely loved that child too. so she wont feel that this is some random kid handed to her.
She craves being a mom so much that she cant see anything off now. and newborn baby mein actually koi features nahi dikhte. they just said it for effect because she is emotional
she actually got gut feel that premie itni jaldi out of observation kaise agaya
i'm sure there will be other moments where she may sense something is on but her love for the child and maternal instinct will trump that

Tab toh maximum DDP milta. Being aware of her own loss and having to live the life she could have had, knowing that one day it was all gonna be gone. Rohit sa is useless sa x100.

They could have done that without this swap thing. Like Abhira could have instinctively tried to stop the baby from crying or something, and Ruhi would have have made a scene about haqs.
This step of Rohit (and eventually armaan) would do nothing but make Abhira go through the pain of losing two babies, only because Rohit sa couldn't handle the responsibility of being a father.
