Currently ruhi is definitely not positive but seeing history of RS whitewashing every bad deed of characters and portraying them perfect after few days, Ruhi can become good and caring sister after certain time.

That armaan-ruhi sequence gave me vibes as if armaan knows ruhi is mental right now and this is why he's not saying anything to her and agreeing to her every demand. Perhaps in future they will show a flashback of armaan meeting ruhi's therapist to show how ruhi condition is very critical
